No, you don't, but please write "Functional Again" on the sticker
that says: "*MUST BE COMPLETED**
Please do the tests as close as possible to the following Monday. Have it ready to be
couriered on the Monday morning.
Tick only the top box (OAT) please, unless it has been discussed with your
Practitioner. The other options will add extra costs to the test that might not be
required.
Please do the 3 day test unless your practitioner has given you different
instructions. The see through bags are marked Day 1, Day 2 and two Day 3 bags. On
day 3 please fill the 3 vials in the two bags. Please fill all the (5) vials to the line
marked on the vials. Only the vial with the white lid in one of the Day 3 bags and
the white little Ice-Brix go in the freezer. The rest of the vials go in the fridge.
If you are only doing 2 days the first day can be either Day 1 or Day 2, but you have
to include the two Day 3 bags.
Everything goes back into the box and then into the Courier bag to be couriered.
